Health Insurance & Payment #
CISI Policy and Coverage #
The Swedish Program provides health insurance from CISI (Cultural Insurance Services International). The insurance is specifically designed for students living abroad and is valid anywhere outside of the United States. In general, the policy covers medically-necessary treatment for acute and emergency healthcare. Read through the policy carefully to familiarize yourself with your coverage and exclusions.

CISI ID Card #
After submitting your registration forms and tuition deposit to The Swedish Program, you received a welcome email from CISI. This email includes your ID card, policy brochure, proof of insurance for your residence permit application, and other important information. Whenever you visit a healthcare provider, have your CISI ID card accessible. We strongly recommend downloading these documents to your phone and/or printing hard copies for your wallet. You can also download your insurance documents through the “CISI Traveler” app (see page 23).

Payment: Minor Injuries or Illnesses #
The Swedish Program’s healthcare partners (Dr. Daria Eriksson and associates, Svea KBT, Eva-Mari Thomas, and Dr. Angela Marré-Lippitz) have payment agreements with CISI wherein they can bill CISI directly for your care. This means you do not need to pay anything out-of-pocket as long as you provide your insurance ID card and any other information needed for billing. If you fail to provide the information neededfor billing, you will need to pay for your care upfront. You can submit a claim to CISI for reimbursement as described below.
If you visit any other healthcare provider while you are abroad, ask the provider if they are willing to bill CISI directly. If so, your CISI ID card lists the billing address and contact information for filing a claim. If the healthcare provider is not willing to bill CISI directly, which is the norm for foreign travelers, you will need to pay out-of-pocket and submit a claim to CISI for reimbursement. Be sure to save all receipts and records related to your care. To file a claim for reimbursement, fill out the claim form included in your policy brochure and send the completed form, along with copies of your receipts, to claimhelp@mycisi.com.

Payment: Serious or Emergency Care #
If you require expensive treatment or hospitalization, please contact CISI’s 24/7 emergency assistance provider, AXA Assistance, by calling +1 (312) 935-1703 or emailing medassist-usa@axa-assistance.us. They will do their utmost to arrange direct billing with the hospital.
